Insurance: 

 

We accept several major insurance plans including:

  • Medicare

  • United

  • Oxford (Freedom and Liberty)

  • United Medicare

  • Cigna

  • Empire Plan

  • Blue Cross Blue Shield

  • Call our office to confirm participation since there are some exceptions to these plans

Billing and Payment Information: 

 

Insurance copayments and any balances are to be paid at the time of the visit. There is an additional $15 dollar fee for copayments that are not paid at the time of the visit.

Please be aware that many insurance plans have deductibles and coinsurance requirements. If you have questions about your plan we encourage you to contact your insurance for the most accurate information.
